The Opportunity:
The Home Care Specialist will be responsible for conducting and reporting on all direct care activities for assigned clients to the Founder (or designated leader), and providing support for other assigned business activities. This includes but is not limited to assisting with ADLs, companionship, personal hygiene, restorative/respite care, transportation and self-determination support. The Home Care Specialist will ensure that client care needs and company requirements are promptly met in compliance with all internal standards and local regulations.
Main Responsibilities:

Assists the client with activities of daily living, including bathing, dressing, grooming, toileting, transferring, getting to and from activities, taking/making meals according to the individual service plan

Transporting clients to and from social activities or nearby appointments (subject to client service plan approval). Accompanying a client to such appointments/activities in a vehicle driven by the client is permissible

Promotes client independence through encouragement to participate in their own care while maintaining safety

Serves meals to the client and may assist in preparing meals

Encourages the client to eat a proper diet and take adequate fluids while respecting the client’s right to free choice; records and reports changes in the client’s eating habits to the supervisor

Initiates and participates in leisure activities provided for the client as described in the activity calendar and encourages the client to socialize and participate in planned activities or programs to develop friendships with other clients/individuals

Maintains the client’s records daily in a timely manner and in accordance with company policies, procedures and applicable regulations; documents medication distribution as applicable, leisure activities, incidents, and observations; reports any changes in the client’s physical condition and/or behavior, and visitors; observes and reports the health and emotional condition of each client; and promptly reports all changes to the supervisor

Maintains a clean, safe, and orderly environment for the client; performs general housekeeping essential to care in the home; and follows cleaning schedules for laundry, bedrooms, dining area, living space, bathrooms, kitchen, and other areas

Follows proper procedures in emergency situations and responds promptly and positively to the client’s requests for assistance while also providing comfort and companionship

Encourages teamwork through cooperative interactions with fellow employees

Participates in the care planning of participants through cooperation with the multidisciplinary teams

Attends internal and continuing education training/programs/meetings to stay up to date on changes in approach to client care and to maintain required certifications

Required Candidate Qualifications:

Registered (or willing to register) in the California Home Care Provider database

You’ve had a job or experience in a position with similar responsibilities
-
High School Diploma/GED or equivalent role specific training
-
Possess a valid driver’s license

Willing to take/show completion of recent TB test & Live Scan
Preferred Candidate Qualifications:

Live Scan Clearance

CPR, AED & First Aid Certified

Multilingual

Willing to work weekends
